Positive Hepatitis C Serology

When hepatitis C serology is positive, order HCV RNA to confirm active infection.

Chronic Hepatitis C (HCV RNA+)

For patients with current hepatitis C infection (HCV RNA+), follow these steps:

  1. Report to Public Health - Contact AHS Communicable Disease Unit
  2. Post-diagnosis counselling
  3. Complete history - Risk factors, alcohol use, etc.
  4. Physical exam - Signs of liver disease
  5. Screen contacts
  6. Vaccination - Refer for pneumococcal and hepatitis A/B vaccination if non-immune
  7. Order abdominal ultrasound
  8. Hepatology referral - All chronic hepatitis C patients should be seen to plan for treatment with direct-acting antivirals (DAAs)

Baseline Investigations

We usually order these baseline investigations:

  • HCV genotyping
  • Liver function tests
  • CBC
  • INR
  • Hepatitis A and B serology
